Evergreen Sports Medicine, a division of the Evergreen Family Health Group (EFHG), is an independent, physician owned and operated specialty clinic. We focus on top-notch, accessible health care in a collegial work environment. When you join the team at Evergreen Sports Medicine (ESM), you will make an impact on the lives of active individuals of all age ages working in collaboration with board-certified sports medicine providers to help deliver high quality, evidence-based patient care.
Evergreen Sports Medicine was established as part of Evergreen Family Health in 2017 and has since served thousands of patients throughout the state of Vermont and eastern New York. Over the last 6 years, ESM has grown from a single-practitioner clinic to having 3 busy Sports Medicine practitioners. We have added state of the art imaging with on-site digital X-ray as well as point-of-care ultrasound for diagnostic and therapeutic applications. ESM is focused on improving patients’ access to healthcare by offering same or next-day appointments for traumatic and acute injuries, helping to keep patients out of urgent care and the emergency department. We also strive to offer all patients an appoin...tment within 1-2 weeks for their subacute or chronic sports medicine conditions. This model creates a fun and exciting dynamic for practitioners and has been well-received by referring providers and patients alike.
